Purpose

We will take some time upon the completion of each step of the CCJF to reflect and practice the information you’ve gained. Self-reflection and deliberate practice are two ways to develop your clinical judgment skills to transition from a novice to advanced beginner nursing student. Stopping to reflect and practice new skills you’ve learned before moving on to the next step of the framework supports information retention and provides cognitive scaffolding for subsequent steps in the framework.

Task

Use the Flip interactive video program embedded below to apply your learning of the first step of the CCJF, Getting the Information to the following case study:

Getting the Information: (NP/Assessing) (NCSBN/Recognizing Cues)

You receive a report from a Post-Anesthesia Care Unit (PACU) nurse about Mindy Johnson, a 60-year-old woman who is being transferred to you on a busy medical/surgical unit. Ms. Johnson had an open cholecystectomy surgery without complications and has been stable in the PACU. She received fentanyl (Sublimaze) 50 mcg IV 30 minutes ago.  The PACU nurse states the patient has been complaining about her level of pain and states the patient is being “overly sensitive and unrealistic about where her level of pain should be after surgery.”

Ms. Johnson’s medical/surgical history includes:

  • diabetes mellitus type 2
  • diabetic neuropathy with chronic pain
  • hypertension
  • hysterectomy

Ms. Johnson’s home medications are as follows:

  • metformin (Glucophage) 500 mg po BID
  • glipizide (Glucotrol) 5 mg po daily
  • hydrochlorothiazide 25 mg po daily
  • amitriptyline (Elavil) 50 mg po daily at HS

Ms. Johnson’s lab results are as follows:

  • Hemoglobin: 12% 
  • Hematocrit:  37%
  • Glucose:  120 mg/dl

You settle Ms. Johnson in her room and complete an initial head-to-toe assessment and identify the following findings:

  • BP 162/88 mm Hg, heart rate 110, respirations 20, pulse oximetry is 98% on room air
  • Alert and Oriented
  • Grimacing, moaning, and guards her incision
  • States she is in severe pain
  • Dressing is intact
  • Lung sounds are clear and diminished in the bases
